Credit Card authorization form for on-location stylist

As a policy of our Salon, prior to any stylist going out on location, we are requiring a credit card to be held on file.

This Credit Card will Not be charged if you decide on the day of your appointment that you would like to pay with a different form of payment (Zelle, Venmo, credit card).
